It is absolutely possible! This Bluetooth SIM Carrier comes with 2 SIM Slots (Mini SIM, not Micro SIM) that when connect with iPhone 4 / 4s, then it will be a 3SIM-3Standby iPhone 4 / 4S!
You can call via anyone SIM among the 3 SIMs (2 in the SIM Carrier, 1 in your iPhone 4 / 4S) and accept call from any SIM whenever the iPhone is available and in standby mode. (see note below).
For iPod Touch and iPad, the scenario is exactly as iPhone 4 / 4S, just it does not have a built-in SIM inside but it still makes iPod Touch 4th and iPad 2/3 a dual-SIM-dual-standby phone!
