The logic is so simple, just use a threaded photo cover for iPhone 5 with a adapter ring for 37mm lens, then you can use any 37mm Lens (see notes below) from your DSLR on your iPhone 5.
The advantages are that you can share those 37mm lens for your SLR or DSLR and and the same time, taking pictures in a even higher quality as those lens for SLR is much higher than those for iPhones or Cellphones.
